Seems to be a semantic debate. We can define words however we want to. Are we talking about sex or gender? Are we talking about biology or identity?

I happen to think all self identity is in our heads. Where else could it be? If you are a Muslim, you're a Muslim in your head. If you are a Christian or an Atheist or a Liberal or a Conservative or a Sportsball fan, where else are you such a thing but in your head?

So it doesn't shock me that some people identify as a woman/man or a cat or British in their heads.

Consider religion. They can't all be right, so at least some, if not most, religious people hold some beliefs that we could justifiably call delusional, or not corresponding to reality. But we don't send people to insane asylums or have them take medications or seek therapy. We have a gentleman's agreement to respect that set of delusions and allow people to live them out in relative peace.

So it isn't quite as simple as saying, 'yes, but these people's self identity is delusional'. We accept some delusions. And perhaps we are all delusional about some aspects of our self identity. I am probably not as smart, funny or good looking as I think i am.

So maybe it is more a matter of what delusions do we accommodate and what should our criteria be for which delusions deserve reasonable accommodation - the kind we afford religious delusions, political delusions, delusions of ego/grandeur etc.

And that's before addressing the arbitrary nature of social norms we have set up around gender. Maybe if we didn't enforce such strong, arbitrary norms, we wouldn't have people who don't fit those norms identifying as other genders.

I think the turning point was Global Warming. Although some on the Left have already disagreed with me on it, Gore put too much politics into his global warming alarmism. The reaction from the right was even more politicization. As the nation has seen happen over the post 20 years, this bounced back and forth like ping-pong ball until the Right leaned toward anti-science and the Left toward pro-science.

While I agree too many Americans worship the Almighty Dollar, beginning with #PABOTUS and down to our resident RW minimum wage beta males like Darth and Don, there are strict rules on sex change operations.
