I have the scar. Smallpox has
~30% fatality rate. Since routine vaccinations ended in 1972, it became a significant concern after 9/11 when the idea of a bioweapon came up. Nukes are flashier (literally) but effectively spreading smallpox in NY or LA would be as disastrous as a dirty bomb. At the time, the US didn't have enough smallpox vaccine for the population. A third would be dead before it could be made. I have no idea what the status is now.
