It's kind of funny now to think about how conservatives hyped this debate for months: first, that there was no way Biden would show, and then if he did, he'd be all doddering and incoherent.

Trump really flipped that script. He was completely unhinged. Biden wasn't great, but he was fine. No one is talking about him today, though. The debate was about Trump's meltdown, and that's how it will be remembered.

Disagreed. Biden is a professional and came prepared both intellectually and physically (e.g. rested). That helps even a 77 year old man look good.

Trump just ranted the same bullshit conspiracy theories he's been screaming for six months. It's a good thing I wasn't in a drinking game with Trump saying words like "Hunter", "China" and "Democrats" or I would have been shit-faced after 15 minutes.

The debate was a shitshow and I saw little coherency out of it.

There’s neural damage that occurs that then impairs the function of these neural transmitters, these chemicals like acetylcholine, which is this chemical that’s really responsible in large part for our normal cognitive function. And so someone with dementia would not be treated with amphetamines to get them more alert. It wouldn’t increase their brain acuity at all. It would make them jittery, more confused, really, because they wouldn’t be able to think clearly. Drugs for dementia are primarily aimed at increasing levels of acetylcholine, like Donepezil, which is used for Alzheimer’s dementia.

So just to be clear, you’re saying that Adderall is not a treatment for dementia?

There are a number of treatments including methylphenidate for general apathy associated with dementia.

Considered a distinct entity from depression, apathy is common late in life, particularly in persons with dementia of the Alzheimer’s type (DAT); 70% to 90% of patients may experience apathy at some stage of dementia. Apathy is linked to impairment in activities of daily living and needing more assistance from caregivers, which increases caregiver burden. Treating apathetic symptoms may improve quality of life for the patient and caregivers.

Background:
Despite its importance in the lived experience of dementia, symptom fluctuation has been little studied outside Lewy body dementia. We aimed to characterize symptom fluctuation in patients with Alzheimer's disease (AD) and mixed dementia.

Methods:
A qualitative analysis of health records that included notations on good days and bad days yielded 52 community-dwelling patients (women, n = 30; aged 39–91 years; mild dementia, n = 26, chiefly AD, n = 36).

Results:
Good days/bad days were most often described as changes in the same core set of symptoms (e.g. less/more verbal repetition). In other cases, only good or only bad days were described (e.g. no bad days, better sense of humor on good days). Good days were typically associated with improved global cognition, function, interest, and initiation. Bad days were associated with frequent verbal repetition, poor memory, increased agitation and other disruptive behaviors.

Conclusions:
Clinically important variability in symptoms appears common in AD and mixed dementia. Even so, what makes a day “good” is not simply more (or less) of what makes a day “bad”. Further investigation of the factors that facilitate or encourage good days and mitigate bad days may help improve quality of life for patients and caregivers.

It does not typically happen for as long as 90 minutes, nor can it be commanded on demand. It has nothing to do with what drugs the patient is taking or what foods they ate or what stimulus they have received. It is totally unpredictable. One common factor though, with dementia symptoms, is nicknamed "sundowners" by caretakers. As light dims and sunset approaches, dementia patients typically become more confused and disoriented. Bright lights can counteract this phenomenon. You know -- like on stages.

Biden does not have dementia. I believe -- from observing his gait, physical mannerisms, struggles with speech and repetitive use of simple words and phrases, and inability to attend for long -- that Trump is in the early to more likely moderate stages.

So go ahead and vote for the mentally-declining racist bully who can't even manage to make a taxable profit in 10 out of 15 years of allegedly being a "billionaire." Vote for the spreader of hate and division. Vote to end America.

Agreed. I'm not an expert, but Biden strikes me as someone who is an average 77 year old man in terms of brain function; another reason why I think both he and Trump are too old for the job. Additionally Trump seems to have mental issues.
