It is not an impediment for some people.

For others it is.

There is a medical condition know as alcoholism, in which case the subject is allergic to alcohol and suffers ride ranging negative consquences, when they imbibe. Being that alcoholism is an addiction, it can be dificult to impossible for an active alcoholic to stop drinking.

Alcoholism is a progressive fatal disease. The medical success rate for curing the disease in the low single digits.

Even people who are not alcoholics are often killed by alcohol.
That it is socially acceptable just adds to the malady
