When you are creating a new thread you select the box next to "create poll with this post" and the number of question will be in your poll. It will redirect you to add your poll question and your answers when you hit submit...
Yeah, that is how I created this one. You can also make them single-selection and there are other choices... Like I could have made it show who was voting for which selection...
