Cocaine and fentanyl are both already illegal. People arrested for their use that go to prison have to take drug treatment programs. The recidivism rate on those is quite high.
So, what regulations and treatments are you proposing?

They also know who is carrying what at what time INSIDE the U.S. They have known for 15+ years. They choose not to interdict the shipments inside the U.S. on our soil, in most cases. This limits collateral damage, particularly the children and women of the Narco-Traffickers.
When they intercede in International Waters, or the Airspace above it, the people killed are usually only the worst of the Narco-Traffickers, the adult males who are armed and violent.
