US Transportation Secretary Sean Duffy expressed grief over the tragedy, sharing “heartbreaking images coming out of Kentucky tonight.”

He added that the FAA and NTSB teams were mobilising to investigate. “Please join me in prayer for the Louisville community and flight crew impacted by this horrific crash,” Duffy said.

Reported fatalities and injuries: At least four people have died and 11 were injured, police said Tuesday, warning those numbers could climb as the investigation continues. Kentucky Gov. Andy Beshear said earlier some of the injuries are “very significant.” Louisville Mayor Craig Greenberg wrote on X this evening that the city has “every emergency agency responding to the scene.”

• Status of crew unknown: Beshear said the status of the crew on board the plane is still uncertain and they are working to reach impacted families as soon as possible. “Anybody who has seen the images in the video know how violent this crash is, and there are a lot of families that are going to be waiting and wondering for a period of time,” he said.

Crash investigation: The Federal Aviation Administration and the National Transportation Safety Board are investigating the crash of UPS flight 2976. The NTSB will lead the probe, the FAA said Tuesday. The safety board announced it is launching a “go-team” to investigate, which is scheduled to arrive in Kentucky on Wednesday.

It was a severe mechanical failure, but it's possible that a shortage of air traffic controllers might have prevented the pilots from being notified to abort the takeoff in time.

I'm sure the tapes will be reviewed along with the "black boxes". What bothers me is that, under the Trump administration, they tend to bury anything that makes them look bad.

I screwed up and typed Lexington when the crash was in Louisville.

Regulations on cargo operations are much less stringent than on passenger airlines....not that it helps the people on the ground who were killed when an MD-11 full of fuel plowed into them.

Trump's shutdown and the incompetence of his cabinet are mostly contributing to slowdowns and inefficiency of operations, not necessarily compromising safety....at this time.

The fire on engine #1 was catastrophic.. an uncontained failure that sent debris flying uncontrollably... but the real killer was the immediate compressor stall on #2, visible right here in the footage as that telltale puff of flame.This almost certainly resulted from Foreign Object Debris (FOD) ingestion, where fragments from #1's explosion were sucked into #2's intake, disrupting airflow and surging the compressor blades. Watch the sudden yaw to the left post-stall: that's the asymmetric thrust kicking in, with only engine #3 (tail-mounted) left producing power. For a fully loaded MD-11 like UPS Flight 2976, heavy with cargo and fuel for its route, climbing out on a single engine was impossible..performance data shows these birds need at least two for safe ascent under max takeoff weight.Pilots did everything by the book; ATC audio suggests they declared emergency instantly. No procedural tweak could've changed this outcome. RIP to the three lost on board, thoughts with the injured ground personnel.
 
News reports and video show an engine by the runway and the left side of the aircraft on fire.

This might indicate a similar scenario to the 1979 DC-10 crash in Chicago where the left engine departed the aircraft and took out hydraulic lines and disabled flight controls resulting in an uncontrollable aircraft. An MD-11 is an updated version of the DC-10.


The National Transportation Safety Board (NTSB) found that as the aircraft was beginning its takeoff rotation, engine number one (the left engine) separated from the left wing, flipping over the top of the wing and landing on the runway. As the engine separated from the aircraft, it severed hydraulic lines that lock the wing's leading-edge slats in place and damaged a 3-foot (1 m) section of the left wing's leading edge. Aerodynamic forces acting on the wing resulted in an uncommanded retraction of the outboard slats. As the aircraft began to climb, the damaged left wing produced far less lift than the right wing, which had its slats still deployed and its engine providing full takeoff thrust. The disrupted and unbalanced aerodynamics of the aircraft caused it to roll abruptly to the left until it was partially inverted, reaching a bank angle of 112°, before crashing in an open field by a trailer park near the end of the runway. The engine separation was attributed to damage to the pylon structure holding the engine to the wing, caused by improper maintenance procedures at American Airlines.
