PSA does not have everything to do with a cancer diagnosis, BUT Brandon had plenty of other symptoms, so this was no surprise. A nodule on his prostate could very likely have been caught during a DRE at his normal physical.

When the PSA starts to rise rapidly it is a good indicator for more tests like a MRI or biopsy. Biden was probably getting aroused during his DRE.
