In the populations you cite, most are still accepted WITHIN THOSE COMMUNITIES. If it's a prison population, or concentration camp - they have acceptance w/ their peers, friendships, support. There isn't the stigma that transgenders have to deal with.

Another layer would be the idea of hope that most in those populations have. Maybe one day they'll get out, or escape, or whatever it might be.

Transgenders don't have that. They can't escape the stigma that society has thrust upon them - as evidenced by your initial argument that they simply have "serious mental issues." Affirmation and indulging them in cosmetic surgery, use of pharmaceuticals, and genital mutilation don't fix those problems. That's why so many transition back later in life when they resolve those issues.
