I would think he would be saying whatever his most severe symptom was or the thing that might be most persuasive in those moments as his life is draining from him.

If it was me, after the first ten times telling him I couldn't breathe, I would throw all caution to the wind and not worry about possibly 'making him mad.' I wouldn't feel I had anything to lose at some point.

If he seemed unmoved by cries of "I can't breathe," I might have wanted to try: 'Officer, this is the BIG one. I'm having a heart attack. My chest pains are bad.' 'You are going to kill me.'
