From what I had read, the US pulled out first; because they figured that the conference was going to be used to try and force an issue.
Evidently Germany thought the same and then it does appear that it occured.

Most of the walkouts happened during the first few minutes of the Iranian President's speech attacking Israel. I believe that the US had already planned to not attend.
