Why does the USA have the worst healthcare insurance in the World.................

Because that is what Barack Obooba forced down the American throat. Obama owns US healthcare, and all of it's problems because even the ones that he did not create, he made worse
 
But many of the uninsured are uninsured on purpose as they chose not to be a part of it. Should they be forced to participate in something they believe is trash

The USA remains a free country.
If someone doesn't want affordable healthcare coverage ... they don't have to take advantage of it.
 
The ACA has saved many millions of lives and provided healthcare coverage for many millions that had never been able to afford it before. The ACA is not perfect. No plan whatsoever will ever be perfect. But the ACA is the very best healthcare system that the USA has ever had in it's history. How can we as citizens encourage our legislators to improve it? The donnie spoke often about our healthcare systems during his campaigns. I totally agreed with most all he said about how he wanted to see in changes to the ACA. Obviously our congress was not listening and have no intentions whatsoever to abide the wishes as verbalized by lil' donnie. But all is not lost. There are several repub senators now that are demanding a welcome to the Democratic minority for future deliberations. A bill of this enormous impact must have a bi-partisan approach. That is only reasonable. Don't you think?

So why are people opting out if it is so great
 
The ACA was designed for those that have adequate coverage via other means to be able to opt out of the ACA. What's wrong with that? In addition, there are several states that do not offer the ACA in any context. There's a lot wrong with that. I repeat. The ACA is not perfect but it IS the best healthcare system that has ever been experienced in the USA.

How many people would be booted off of medicaid under the new GOP bill?

Again I know this is complicated, but there is no GOP bill, it's a sham designed to go nowhere. The real bill comes out in 2019 before the next election, or when the dems beg forgiveness

200,000 people who had no insurance got Obamacare.........250 million people lost their insurance coverage to make it happen.....

You must really have some shit for brains, asshole. Why do the other millions of Americans also have the identical exemptions? Adequate healthcare coverage is adequate healthcare coverage no matter where or what program that you might be enrolled in. Thousands of American businesses made the switch to the ACA because it was more cost effective for their employees. Why did that happen? That's just a rhetorical question as I am certain that you really don't have a damned clue.
