It could happen. Some presidents have lost control of both houses in mid-terms. I had an error earlier, the president's party has lost seats in 36/39 mid-term elections since the Civil War (not 56/59). The average seats lost is 26 since WWII. Democrats need 23 seats to win the House. Clinton lost 60 seats in 1994 (House + Senate), Bush lost 36 in 2006, and Obama lost 69 in 2010.
It all depends on turn-out.

The 2016 polls were fairly accurate. The Real Clear average was Hillary 46%/Trump 44%. The election result was Hillary 48%/Trump 46%.
The actual poll simply shows national vote totals at the time the poll was taken. The prediction about the electoral college was from the writers making predictions to get catchy headlines.
