I expect that those 23% are merely the fake/spam accounts that are so badly managed that they can be easily screened by a simple algorithm. There's probably twice that many fake accounts there that merely aren't so blatant about it.
If you spend much time on Twitter, you start to see a pattern among followers of certain accounts, where a big chunk of people tweet hit-and-run contributions that give no sign of even understanding what the thread is about, much less engaging with any follow-ups. In my experience, the phoniest Twitter armies belong to Musk, Putin, Matti Taibbi, Glenn Greenwald, Tulsi Gabbard, and Zack Snyder.
