BTW, I've flown a GCA. We have one of the few here in Washington. It's rather fun. It ends at a podunk runway little wider than a bike path (but it DID have a centerline down it!). An airline even uses it regularly!

The radar track of both aircraft. It appears the Blackhawk was straight and level at 400 feet following the river southbound and the CRJ was in a descent for landing northbound.

Watching Ted Cruz today repeat about the Army helicopter being too high and a further revelation about PAT 25's ADS-B being "turned off" was also interesting because it reflects Flat World thinking. 2D. Two dimensional thinking.

Those working in aviation work in three dimensions. This accident wasn't just a problem in vertical separation, it was also a problem in lateral separation.

The controller followed the rules.....the rules of operation of this airport assume that the military copter pilots have some competence and are honest and are not doing terrorism.....in this case at least one of these assumptions was wrong.

According to the “best quality flight track data” available, the helicopter was flying at about 300 feet at the time of the midair collision with the plane on its approach to Ronald Reagan Washington National Airport outside Washington, DC, according to a statement Tuesday evening from the National Transportation Safety Board.....

The Sikorsky UH-60 Black Hawk, based at Fort Belvoir, Virginia, was flying a training mission in a dedicated helicopter route where it was not allowed to fly above 200 feet, according to a published FAA chart. Since the accident above the Potomac River, the Federal Aviation Administration has indefinitely suspended the use of most of those routes.....

 
The crash avoidance system on the Helicopter was off and they were using night goggles.
This was a very old not upgraded copter....it did not have a crash avoidance system, and we dont know that they were using Googles but they likely were, which are designed for low light situations......the bright lights in this situation may have made using them here a bad idea because the lights are blinding, and because they limit field of vision.

If it wasn't understaffed there would have been 37% more ATC available that night.

I have not seen that, but the reason there was one not two is 100% on the supervisor.
